Shift Fertility Method Program Contract

This Contract is legally binding. It is your responsibility to read this Contract carefully prior to your use or access of any of our Programs, Products, and Services collectively referred to moving forward as PROGRAM(S).

PROGRAM DESCRIPTION: Your purchase includes the following services:
Twelve (12) Week Program
Twelve (12) weekly content modules
Six (6) biweekly 60 minute coaching calls
Support from Jacy O'Neill via private messaging
25% Fullscript discount for one (1) year

PRICING: The full cost of the program is $1,497, which may have applicable discounts. You have the option to pay in full or choose one of two payment plans: six (6) monthly payments or three (3) monthly payments depending on the option you selected at checkout. Payment plans will have the full amount due evenly split into either six (6) or three (3) monthly payments depending on the option you choose.

REFUNDS: We will do everything within our ability (and within reason) to ensure your satisfaction. Refunds will not be issued for services already rendered. Programs and courses have a 14 day refund period. Refunds must be requested by email no later than 11:59 PM MST on the 14th day after purchase. If you have any questions or concerns, or if there is anything we can do to make your experience a more pleasant one, please email Jacy at hello@shiftintegrativemed.com.

PAYMENT PLAN AND LATE PAYMENTS: Fees for this program are due in full, or if you signed up for a payment plan, according to Our payment plan. By signing up for a payment plan, You authorize Our continued access to Your financial information stored by Our third-party financial processing company, Stripe, until Your payment plan is complete. Late payments are subject to a 10% interest charge per month for any outstanding sum left unpaid. For all payments that are more than 60 days outstanding, Shift Integrative Medicine reserves the right to send You to collections, upon which You will owe the total amount of all outstanding payments, plus any collection costs including reasonable legal fees. In the event you chose to dispute a transaction with your financial institution, we reserve the right to present proof of Your access to the PROGRAMS and this Contract to the financial institution investigating the dispute.

The PROGRAM will not be used in place of my physician’s, therapist, or psychologist recommendations. You also understand that while the Company makes every effort to make accurate statements Shift Integrative Medicine cannot guarantee that all materials are accurate and up to date.
